Job Description Job Description The position is located in
Sells, AZ. Position Summary: Under general supervision, provides
medical care and treatment to clients/patients according to
policies, procedures, protocol and client needs. The work is
normally reviewed upon completion, with occasional spot checks
while in process, to ensure accuracy, timeliness and conformance to
established standards; major work tasks are covered by instructions
or procedures and unusual situations are referred to a supervisor.
Essential Duties and Responsibilities: Prepares schedules of
patients for home healthcare; collects supplies required for each
home visit; maintains inventory of need supplies necessary for the
treatment of patients. Implements individualized patient care plans
according to physician treatment orders. Observes documents and
assesses patient condition; informs primary care providers of any
changes in condition or needs. Monitors and follows up on patients
in serious conditions. Implements treatments and administers
prescribed medications. Assigns cases and provides clinical
direction to home health aides; observes, evaluates and documents
staff performance. Coordinates patient care services with
additional resources, agencies, or hospitals in the community.
Participates in the preparation of patient transfer, discharge
planning and discharge summaries. Collaborates with other Human
Services divisions as well as Indian Health Services in the
efficient treatment of patients. Assists in the development,
implementation, and evaluation of standards of care, quality
assurance, and infection control activities. Counsels and teaches
clients and their families on preventative, curative and
rehabilitative measures regarding their treatment. Submits all
required documentation related to client's care and established
policies, procedures and regulations. Maintains professional
knowledge by conducting research, attending seminars, educational
workshops, classes and conferences; reviewing professional
publications; establishing networks; participating in professional
societies; conferring with representatives of contracting agencies
and related organizations. Performs other job related duties as
assigned and contributes to a team effort. Knowledge, Skills, and
Abilities: Knowledge of the Tohono O'odham culture, customs, and
traditions. Knowledge of applicable federal, state, tribal laws,
regulations and requirements; including HCFA, Medicare, ALTCS, and
AHCCCS. Knowledge of basic medical care procedures. Knowledge of
psychiatric medications and side effects. Knowledge of diabetes and
how it affects mental health. Knowledge of evaluation plans and
treatment of clients with a variety of illnesses. Knowledge of
communicable diseases, including their transmission and prevention.
Knowledge of HIPAA related rules and regulations. Skill in
analyzing problems, projecting consequences, identifying solutions,
and implementing recommendations. Skill in patient care. Skill in
the use of medical equipment utilized in area of assignment. Skill
in providing superior customer service to external and internal
customers. Ability to be compassionate and have good communication
skills. Ability to deal with stress. Ability to carry out
instructions furnished in verbal or written format. Ability to
communicate efficiently and effectively both verbally and in
writing. Ability to exercise independent judgment. Ability to
maintain privileged confidential information. Ability to establish
and maintain positive and effective working relationships with
other employees and the general public. Ability to work
independently and meet strict time lines. Ability to travel within
the interior/exterior boundaries of the Tohono O'odham Nation.
Ability to provide training in Cardio Pulmonary Resuscitation and
First Aid. Minimum Qualifications: Licensed by the Arizona State
Board of Nursing as a Practical Nurse and two years' nursing
experience working with communicable diseases or related medical
care and treatment. Licenses, Certifications, Special Requirements:
Must obtain certifications in Cardio Pulmonary Resuscitation (CPR),
Automated External Defibrillator (AED) and First Aid within the
first six months of hire. Must be licensed to practice nursing in
the State of Arizona and certified by the State Board of Nursing as
a Licensed Practical Nurse. Must have a current immunization
statement to include, proof of a non-reactive tuberculosis skin
test within the last year, proof of immunity for hepatitis B
(Documentation of immunization does not prove immunity, only a
titer with a positive result is proof), within three months of
hire. Upon recommendation for hire, a criminal background and
National FBI check is required to determine suitability for hire,
including a 39-month Motor Vehicle Record. May require possessing
and maintaining a valid driver's license, (no DUIs or major traffic
citations within the last three years). If required, must meet the
Tohono O'odham Nation tribal employer's insurance requirements to
receive a driver's permit to operate program vehicles. Based on the
department's needs, incumbents may be required to demonstrate
fluency in both the Tohono O'odham Language and English as a
condition of employment.
